Required Forms:
Parents should review the following information with their students prior to the start of school. Relevant forms must be digitally signed or printed, completed and returned GHS during the first three days of school.
- Please review the GPS Health Services and Forms page for detailed information on Health Forms and Immunizations, Health Forms required to participate in Athletics and the Authorization for Medical Care Form.
- If your child is a.) new to the District; or b.) is entering middle school (6th grade), please complete the digtial Student Device Loan Form. This is an electronic form where you may indicate that you accept responsibility for any intentional or negligent damage to your child's iPad or Chromebook. You must accept this responsibility if you wish to allow your child to take their device home.
- Press Publicity Permission/Denial Form ** Please print, sign and return this form the the GHS Main Office or the GHS Library Media Center.
